The facility was started in the 1980s by a White father and the Parish Priest of Lwamaggwa Catholic parish. It was started as dispensary offering treatment for simple illnesses for the people of Lwamaggwa Parish and nearby places. It was meant to help people who were travelling long distances to government health facilities by bringing services closer. The facility started operating in the parish hall as Lwamaggwa Dispensary.

In 1998, after visiting Lwamaggwa Parish a German Christian Community from one of the parishes in German decided to support the dispensary by constructing the first Health Centre Building which was later inaugurated by his lordship the Rt Rev John Baptist Kaggwa in 2000 as St Jacob Lwamaggwa Health Centre.

In 2005 the Ann Lange mobile home care building was constructed with funds from German Christian Community and launched by Ann Lange and Hon Edward SSendawula on the 23/06/2005.

The facility is located 45km from Kyotera town in Rakai district, in kooki county, Lwamaggwa sub county, kiweeka parish in Lwamaggwa trading centre
